Global Rates

Gestational Surrogate Cycles

The global rate provides for services for both the patient and the gestational surrogate as described above. The fee for one surrogate cycle is $ 9800 plus pre-cycle costs and additional procedures (if needed) and is paid as follows:

Pre-cycle costs for intended parents-RPMG
	Initial consultation	$ 250
	Program fee/Mandated FDA screening	$ 2,500
	Semen analysis/culture	$ 250

Pre-cycle costs for surrogate-RPMG
	Initial consultation	$ 250
	Mock cycle	$ 730
	Trial Transfer/Sonohysterogram	$ 575

Laboratory costs-Various laboratories
	Basic screening labs (donor)	$ 500
	Screening labs (couple)	$ 150

Surrogate costs-surrogate agency
	Surrogate fee and agency fee	vary

Medications-Pharmacy		vary

Global rate for IVF cycle-RPMG		$ 8,100
Global rate for surrogate cycle-RPMG		$ 1,700

Anesthesia for global		$ 550

First Day of BCPs or Lupron $ 10,350 (global plus anesthesia) plus additional procedures (if needed)

Note: If your cycle is cancelled in any global program, but have paid the full global rate, all services will be itemized only to the point of termination. Any payment on your account that exceeds these itemized charges will be refunded directly to you if you so request, or may be credited to your next cycle.
