IVF and Assisted Reproductive Technology (ART) itemized costs

Itemized Rate

Itemized rates are broken down, usually for the purpose of obtaining insurance reimbursement. Only patients who have pre-certified insurance coverage will have their charges itemized. It is the patient’s responsibility to make sure the insurance pre-certification is obtained. If there is no insurance pre-certification prior to the start of the cycle, the payment-in-advance global policies will be used.

If you have insurance certification and use the itemized rate, you will receive bills from our offices for any remaining balances after your insurance pays. We must receive certification before you start your cycle. If you have a co-payment, it must be paid in advance.

The itemized rate is not eligible for the SUCCESS PROGRAM.

NOT INCLUDED IN ANY ESTIMATE OF THE ITEMIZED RATE:

IVF PRE-TREATMENT PROCEDURES:

Consultations
Pre-cycle lab work including infectious disease screening
Semen testing
Procedures such as trial transfer, hysteroscopy (if necessary)

ADDITIONAL COSTS ASSOCIATED WITH THE IVF CYCLE:

Anesthesia for retrieval (billed by anesthesiologist)
Medications (billed by pharmacy)
Surgical Sperm Retrieval-if needed
Intracytoplasmic Sperm Injection (ICSI)-if needed ~$1750
Preimplantation Genetic Diagnosis (PGD)-if needed ~$5500-$6000
Blastocyst culture ~$600
Cumulus co-culture-if needed ~$300
Embryo freezing ~$550
Frozen embryo storage or frozen embryo cycles-if needed
In donor and surrogate cycles:

Administrative fees ~$2500
Surrogate or donor recruitment, screening or remuneration (billed by donor agency)
Insurance fee for known donors~$500

Pregnancy monitoring following the first pregnancy test
