Global Rates

Combined Gestational Surrogate/Egg Donation Cycles

The global rate can be used when both an egg donor and gestational surrogate are being used. The fee for one surrogate/egg donor cycle is $ 10,300 plus pre-cycle costs (as above) and additional procedures (if needed). It provides for medical services for both the egg donor and gestational surrogate as described above and is paid as follows:

Pre-cycle costs for recipients-RPMG
	Initial consultation	$ 250
	Program fee/Mandated FDA screening	$ 2,500
	Semen analysis/culture	$ 220

Pre-cycle costs for donor and surrogate-RPMG
	Initial consultation (donor)	$ 250
	Initial consultation (surrogate)	$ 250
	Mock cycle (surrogate)	$ 730
	Trial Transfer/Sonohysterogram (surrogate)	$ 575
	Insurance fee for known donor	$ 500

Laboratory costs-Various laboratories
	Basic screening labs (donor)	$ 500
	Basic screening labs (surrogate)	$ 500
	Screening labs (couple)	$ 150

	Donor fee and agency fee	vary

Medications-Pharmacy		vary

Global rate for IVF cycle-RPMG		$ 8,100
Global rate for donor/surrogate cycle-RPMG		$ 2,200

Anesthesia for global		$ 550

First Day of BCPs or Lupron $10,850 (global plus anesthesia) plus additional procedures (if needed)

Note: If your cycle is cancelled in any global program, but have paid the full global rate, all services will be itemized only to the point of termination. Any payment on your account that exceeds these itemized charges will be refunded directly to you if you so request, or may be credited to your next cycle.
